A Permit to Work system is a formal written system used to control certain types of work, which are identified as potentially hazardous.
作業(yè)許可證制度是一種對某些類型的作業(yè)進(jìn)行控制的正式書面制度。

Essential features of a Permit to Work system are:

? Clear identification of who may authorize particular jobs,

? Training and instruction in the issue and use of permits.

Permit Display
Copies of a Permit to Work should be clearly displayed:
應(yīng)該在下列場合清楚地顯示作業(yè)許可證的內(nèi)容：
?  At the work site.在作業(yè)場所。
?  In the permit co-ordination room, with additional copies at the worksite.
     在許可證辦公室，留有作業(yè)現(xiàn)場許可證的副本。
Permit Suspension
If for any reason work controlled by a permit has to be suspended then the permit will immediately be returned to the Superintendent who will acknowledge the fact and co-ordinate the return of all permits to the Permit Office.
如果由于各種原因，原來受許可證控制的某項作業(yè)不得不暫時停止, 則應(yīng)該將許可證立即返還所在部門的主管，再由主管向許可證辦公室通報，并將各種許可證退還給許可證辦公室。
Example of reasons for suspension are:
許可證暫時收回執(zhí)行的理由舉例如下：
?  Site general alarm.現(xiàn)場出現(xiàn)一般的報警。
?  Operational reasons.操作原因。
?  Conflict of work activities.作業(yè)發(fā)生沖突。
?  Expiry of period for which permit is valid.作業(yè)許可證有效期到期。
?  Change in work site conditions.作業(yè)現(xiàn)場的情況發(fā)生變化
After suspension it will be necessary, once permission has been given to recommence work, to have the permit re-validated before the work can restart.
在許可證暫時收回以后，一旦允許重新開始作業(yè)，則在重新開始作業(yè)之前，必須對原頒發(fā)的許可證重新核準(zhǔn)。

When a permit is authorized by site HSE team, it will have a time expiry placed on it. This may vary form a few hours to a full shift, depending upon the nature of the work being undertaken and the frequency at which the activity is repeated.
現(xiàn)場HSE 小組在簽發(fā)許可證時，應(yīng)該注明許可證的有效時間。有效期有可能是幾個小時，也有可能是整個班次，具體要由作業(yè)的性質(zhì)以及作業(yè)重復(fù)的頻率決定。

If, at the end of this time period, the work being controlled by the permit is not complete the permit may be revalidated by the responsible person, who shall ensure, together with the site HSE team, that the original conditions required by the permit are still being met.
如果在有效期到期時，許可證規(guī)定的作業(yè)沒有如期完成，則應(yīng)該由相關(guān)的負(fù)責(zé)人對許可證進(jìn)行重新核準(zhǔn)。負(fù)責(zé)核準(zhǔn)的人員在核準(zhǔn)之前，必須與現(xiàn)場HSE小組一起，確認(rèn)許可證原對應(yīng)的作業(yè)條件是否已經(jīng)發(fā)生變化。
Hand Back
Prior to the permit being signed off and handed back the following must be met:

Training Required
Training on Permit to Work systems is necessary for all persons who may be involved. This should include managers, supervisors, safety engineers of site HSE team, approver of Permit and contractors.

Type of Permit
Permit to Work Certificates required will generally cover the following areas:

?  Hot Work, i.e. welding, flame cutting, grinding.

?  Confined Space work 密閉空間作業(yè)

?  Work involving the use of dangerous substances i.e. radioactive material and explosives.

?  Lifting Operations over 25 Tons, the length more than 23.6m or using two cranes.

Permit to work forms will be individually numbered and carbonized. The white copy will be kept by the PTW Controller. The red copy will be issued for work.
